If applying by email, the following must be adhered to (by email only):
(1) Please put the following subject under the subject field of the email being sent: "V2J Dance Application"
(2) Under the field "Applicant's Signature", please re-type your Driver's License or SIN no. that was previously entered
(3) Please attach an image file of yourself to the email. The image file MUST be no greater than 1MB and no larger than 600x400 pixels in size. Image must show a clear head shot of the applicant.
(4) If the applicant is under 18, under the field labeled "Guardian's Signature", please have the guardian re-type his/her name and relationship.
(5) If at any point you do not feel secure having your information sent via email, then please simply have the application form filled out and sent to the address above.
(6) Applying by email has its risk. Emails may get lost or corrupted during the exchange. The applicant must understand these risks and assume full responsibility should the email not get sent properly. Failure to adhere to these rules when sending your application via email may result in the nullification of the application.
